Beginning April 15 and going through June 15, Mizell Memorial Hospital will be administering a Community Health Needs Assessment (CHNA). The purpose of the survey is to determine the health needs of the community.

The most convenient way to access the survey is through Mizell’s website at www.mizellmh.com. Paper copies are also available at various locations around the community, including the hospital, or one may request a survey be mailed by calling 334-493-9112.

Anyone older than 18 who resides in our community is encouraged to participate. In addition to completing the survey, a place is provided for additional comments and concerns you feel need to be addressed.

The Mizell staff looks forward to hearing from the public and plans to utilize this information to develop a strategic plan that meets the healthcare needs of the community. Hospital officials thanks the public in advance for participating.
